Many people with type 2 diabetes are not able to achieve blood glucose targets with oral noninsulin medications alone. For these patients, exogenous insulin therapy is essential for preventing diabetes-related complications. Diabetes management has been significantly advanced by the development of insulin analogues, which are synthetic preparations that resolve some of the pharmacokinetic limitations and adherence barriers of conventional human insulin therapies. Novel premixed insulin analogues, which require only 1 or 2 daily injections, are designed to control both fasting and prandial glucose excursions.
Until recently, no comprehensive study had been conducted on the benefits and harms of premixed insulin analogues versus other insulin therapies and noninsulin oral antidiabetic agents. In 2008, a comparative effectiveness review on this topic was published with support from the Agency for Healthcare Research and Quality (AHRQ). This educational activity – featuring the first author of the AHRQ systematic review and a leading nurse practitioner who specializes in diabetes care – presents an engaging case-based discussion about the review findings and their potential applications in clinical practice.
At the conclusion of this activity, the participant should be able to:
- Summarize the major findings from AHRQ's systematic review on the comparative effectiveness of premixed insulin analogues for adults with type 2 diabetes
- Explain the tradeoffs in benefits versus harms for premixed insulin analogues, other insulin preparations, and noninsulin antidiabetic drugs
- Demonstrate effective strategies for communicating comparative effectiveness research findings to patients
- Apply AHRQ's systematic review findings to educating patients about options for insulin therapies and to making patient-centered treatment and management decisions
